How to Choose a Face Wash for Acne – Buying Guide

Before choosing a facial wash for acne, the dermatologist must be able to diagnose your condition if it's really acne. There are lots of skin conditions that may mimic acne vulgaris (or common acne), among these are Rosacea, Perioral dermatitis, or Steroid-induced acne depending on your history and clinical presentation.

Since skin quality and type differ for each person, it's important to know which acne facial wash will be beneficial for you. We highly recommend consulting with a board-certified dermatologist for proper assessment.

Check the Type of Acne

Three different groups of acne patients may also be considered: pre-adolescent, adolescent, and post-adolescent. Post-adolescent acne may be further subdivided into persistent or late-onset type. It's important to note that the severity of acne vulgaris may appear at various stages of life.

There are different types of acne depending on the history of the patient. Numerous conditions may mimic acne vulgaris; proper assessment is highly suggested before applying over-the-counter products

Once assessed and the final diagnosis of acne vulgaris is given, your condition will be further classified according to severity: mild, moderate, and severe, depending on the number of comedones, papules, pustules, and/or cysts.

Puberty Acne: A Gentle Face Wash

Puberty Acne: A Gentle Face Wash
Several changes happen during a person’s growth, and hormonal modifications are one of them. Puberty can introduce an excessive amount of sebum to the skin, and this can cause an increase in acne-causing bacteria.

It’s possible to suppress the bacteria by washing the sebum away with the right face wash. However, keep in mind that the skin can become sensitive during puberty, so it’s best to avoid a strong formula to avoid additional complications.

Also, puberty acne tends to get inflamed when stimulated. Hence, face washes that scrub or peel the skin may worsen the condition. As such, choose a skincare product with a gentle formula for puberty-related acne.

At around age 8 to 9 years old, secondary sexual characteristics appear. This results in increasing dehydroepiandrosterone sulfate (DHEAS) levels, which is converted to more potent androgens, causing the sebaceous gland to expand and produce more sebum. Based on studies, sebum secretion peaks during this time and declines after the age of 20.

Contrary to popular belief that scrubs unroofs closed comedones (whiteheads), its use is highly discouraged as friction would further damage the integrity of the stratum corneum. Facial scrubs or any other type of scrubs are not recommended if you have a sensitive skin type, as you may have the risk of developing contact dermatitis.

Male Acne: A Face Wash With Cleansing and Moisturizing Power

Male Acne: A Face Wash With Cleansing and Moisturizing Power
Male acne is often caused by hormones. Men tend to experience more sebum production than women. Therefore, choose a face wash that can cleanse the skin from the excess sebum

Also, men often experience acne breakouts because of dry skin. As such, searching for a cleansing face wash that can also act as a moisturizer is a must. Moisturizing agents like hyaluronic acid in face washes are excellent ingredients.

Also, male acne is similar to puberty acne in a sense that it can inflame from stimulation so a skin care product with anti-inflammatory ingredients should also be a great option to use.

One of the key pathogenesis in acne is the increase in sebum production. This event plays an important role in the development of acne regardless of gender. Greater sebum production in men may be attributed to the fact that they have increased androgen levels, to begin with. As compared with women, sebum excretion in men decreases with age.

The appearance of acneiform lesions after the intake of supplements containing steroids is called steroid-induced acne and is treated in a different manner.

The common culprit for treatment failure in men is non-compliance because some are less experienced in using topical products. To avoid this, I usually limit medications to once-a-day applications until my patients get used to it.

Adult Acne: A Cleanser with Balanced Formulation and Moisturizing Power

Adult Acne: A Cleanser with Balanced Formulation and Moisturizing Power
Becoming an adult can expose you to different lifestyles and habits, and some of these include things like drinking, smoking, and work-related stress. These events can cause your skin to acquire excess sebum, dirt, and small debris that can clog pores that will eventually turn to acne.

A face wash with a strong formula can help eliminate these unwanted substances from the pores. However, if the formulation is too strong, then it can dry the skin. It’s with this reason that you should get a face wash that not only has a strong formula but can also act as a moisturizer to defend your skin against this issue.

There are 2 types of post-pubertal acne: Persistent acne, which is a continuation or relapse of acne vulgaris from adolescent years into adulthood and middle age. On the other hand, late-onset acne involved patients ages 25 years and older who were not affected by acne vulgaris.

In a retrospective review by Skroza et al., the group concluded that persistent type is more common than the late-onset form. Data on the relationship between acne and smoking still remains to be controversial. However, it has been proven by a number of studies that nicotine can cause an increase in oxidative stress and reduce the level of alpha-tocopherol in the plasma.

Peroxidated lipids have a hyperproliferative effect on keratinocytes rendering them comedogenic. The alpha-tocopherol is an antioxidant that functions to maintain the low levels of peroxidized lipids.

Check the Ingredients That Match the Skin Type

Check the Ingredients That Match the Skin Type

No matter what type of acne you may have, it’s important to get rid of the excess sebum while giving the skin proper moisture. However, since skin type is different for each person, it’s essential to check the ingredients of the face wash to check if it’s suitable for use on your skin.

Facial cleansers contain additives and surfactants, and these compounds can irritate sensitive skin. As much as possible, avoid any face wash with preservatives or harmful chemicals mixed in the list of ingredients.

People with mixed skin types, like a combination of dry and sensitive skin, might find it difficult to choose the right face wash. A safe bet for any skin types is to choose a skin care product without additives that gently cleans and moisturizes your skin.

With the increasing number of cosmeceuticals available in the market, it can be very difficult and confusing for both the patients and consumers to know which product works for them. The science of skincare ingredients is constantly evolving, and to avoid misinformation, we encourage consumers to read materials from credible sources.

A lot of ingredients such as parabens, sulfates, propylene glycol are being villainized by a number of movements. These claims must be backed-up with sufficient evidence to avoid instilling fear in consumers.

Allergic reactions to cosmeceuticals are increasingly observed since a lot of people try to experiment with what they buy online. One way to determine if a patient is allergic to a certain ingredient is by doing patch testing. Still, some of the more practical ways in testing new products on your face is by placing just a small amount on your face and observing for any signs of redness, itching, burning, swelling within 24 to 72 hours.

Match the Face Wash Type with Your Skin Concerns

There are three types of face wash you can find on the market, and these are the following: face soaps, cleansing foams, and face washing powder. Choose the right face wash type suitable for use on the right skin type.

Dr. Julius Gatmaitan
First and foremost, identify what type of skin you have based on skin hydration.

  • For normal/combination skin: Use a synthetic detergent liquid in lotion, cream, oil, or gel formulation to dry excess oil on the T-zone area and to hydrate the other areas.
  • For oily skin: Use a foam cleanser with surfactants to remove the excess oil.
  • For dry skin: Oil, cream, or milk cleansers are preferred as they coat the skin with fatty acids on the skin thereby repairing the skin barrier.
  • For acne-prone skin: Look for a gentle cleanser with benzoyl peroxide and salicylic acid.

Face Soap: Ideal for Mixed Skin and Dealing with Excess Sebum Production

Face Soap: Ideal for Mixed Skin and Dealing with Excess Sebum Production
One advantage of using face soaps is they can give a refreshing feeling because they tend to have high detergent strength. As a result, it can be beneficial for people with oily and mixed skin types

Other options include soaps classified as “pure” or those that have a “soap base” because these products don’t use synthetic surfactants or harsh chemical compounds to make the product gentle on the skin.

These soaps aren’t suitable for use on dry skin, even if they do have moisturizing agents. It’s because they tend to have ingredients that cling to the skin, which can lead to other issues. It’s important to know that you should moisturize properly after washing the face.

There are different types of cleansers: soap, synthetic detergents, and combination bars (modern soap with synthetic detergents).

  • Modern soaps or superfatted soaps are composed of a mixture of tallow and nut oil or fatty acids derived from these products (e.g., those containing stearic acid, triglycerides, lanolin, paraffin, or mineral oil). Due to the alkaline pH of soaps, swelling of the stratum corneum occurs, allowing deeper penetration of soap in the skin, which may possibly cause irritation or itching
  • Synthetic Detergents (Syndets), on the other hand, are composed of anionic surfactants such as sodium lauryl sulfate, sodium laurate, and sodium cocoyl isethionate have neutral or slightly acidic pH that is less irritating to the skin function. They consist of protein-bound micelle like surfactant aggregates provides moisturizing benefit to maintain hydration of the skin.

After washing the face with a cleanser, depending on your skin type, always moisturize right after!

Face Cleansing Foam: Suitable for Dry and Mixed Skin

Face Cleansing Foam: Suitable for Dry and Mixed Skin
Facial foams are among the most common skin care products on the market to properly deal with acne. Many affordable face cleansing foams have moisturizing ingredients that can give the skin proper hydration and care which makes them ideal for dry and mixed skin types.

However, these cleansing foams tend to contain a variety of additives and harsh ingredients to increase the strength of their cleaning and moisturizing powers.

As such, it’s not suitable for use for people with sensitive skin. Even though it’s tempting to wash the skin with plenty of foam and bubbles, use only a face wash with a gentle formula if you have sensitive skin.

Dr. Julius Gatmaitan
Dermatologist
Dr. Julius Gatmaitan

Foam has garnered much attention as a delivery system for topical active ingredients in dermatology. As compared with other delivery systems that leave a sticky or oily film after application, foam delivery does not.

The vehicle base of foam contains the same properties as typical vehicles like creams, lotions, or gels, but in contrast with other vehicles, foams are less dense and have fast-drying effects

Although there is no clinical evidence that foams are superior to other vehicles (creams, lotions, or gels), they are highly preferred by some consumers due to their easy spreadability.

Face Wash Powder: Ideal for Mixed and Sensitive Skin

Face Wash Powder: Ideal for Mixed and Sensitive Skin
Face cleansing powders aren’t as popular in the Philippine market as compared to soaps or foams, but these skin care products feature gentle ingredients that promote low irritation ideal for sensitive skin. Many of these products don’t have additives and other harmful chemicals mixed in their formulas.

However, foaming and bubbling tend to be difficult when using face wash powders, so it can become difficult to use if you’re new to this type of product. Face wash powders are also sensitive to moisture so keep it away from humid places like the bathroom.

Dr. Julius Gatmaitan
Dermatologist
Dr. Julius Gatmaitan

Depending on your skin type, another option is a powder wash. It is up to the consumer to determine what type of cleansing they desire when using this type. In contrast with scrubs that may cause abrasions on your face, powders mixed with water just glides smoothly

If you desire deeper cleansing, less water should be added so that the product is more concentrated.

Most of the time, the powders are made up of pulverized minerals or plant-based powders. Since these are plant-based powders and in the concentrated form, there’s a risk for developing contact dermatitis, similar to other vehicles. If redness, stinging, itching occurs, consult your board-certified dermatologist.

The Correct Way to Wash the Face to Deal With Acne

Using the right face wash for acne can be counterproductive if you wash your skin the wrong way. We’ll help you out in that regard as we show you the correct way to wash your face.

Gently Wash Your Face Twice a Day

Gently Wash Your Face Twice a Day
Start by pre-washing your face with warm water to remove the excess dirt before using the face wash. Pre-washing the face will open the pores to make the most of the cleansing power of the face wash.

Next, firmly wash your entire face using the foam of the face wash instead of just your hands. Thoroughly rinse your face after applying the skin care product to remove any foam, bubbling, powder, or paste that may remain.

While you’re wiping your face with a towel, don’t apply excessive pressure as it might scrape acne which will, in turn, aggravate the condition. Also, washing your face too much can worsen acne so doing the process twice a day should suffice - once in the morning and evening.

Dr. Julius Gatmaitan

Most of the time, we get questions like, "How often should I cleanse in a day?" or "What type of cleanser should I use?" from our patients. Cleansing is an essential part of skincare, which is defined as removing excess dirt, oil, sweat, bacteria, and make-up from the skin.

Cleansing also promotes natural exfoliation by removing dead surface cells allowing better penetration of topically applied medications. It is essential to remove all the soap or synthetic detergents in the skin after cleansing. These products should not remain on the skin for a prolonged period of time as contact dermatitis cases might occur.

Moisturize With a Lotion After Washing the Face

Moisturize With a Lotion After Washing the Face
After washing, moisturize the face with your preferred lotion to prevent the skin from drying. We recommend using a lotion with a milky formula to help trap the moisture in your skin. There are convenient “all-in-one” moisturizers that combine the effects of lotions and other skin care products.

However, it’s always important to carefully check the ingredients of these products and your skin type before using them to avoid complications and issues.

Dr. Julius Gatmaitan
Dermatologist
Dr. Julius Gatmaitan

A moisturizer is a must, regardless of your skin type. In fact, there are three classes of moisturizers: occlusives, humectants, and emollients

  • Ingredients such as petrolatum, mineral oil, cocoa butter, and shea butter are all occlusive agents. Their main function is to decrease transepidermal water loss by forming an impermeable barrier on the skin surface. Occlusives tend to feel heavy and to be greasy on application.
  • Glycerin, hyaluronic acid, and urea are more notable examples of humectants. They draw water from the atmosphere and promote water absorption from the dermis to the epidermis.
  • Ceramides & Squalene/Squalane are examples of emollients. They fill the spaces between the desquamating corneocytes hydrating the skin and making it appear smoother.

What moisturizer may work would depend on your skin type. Dry skin would benefit more from cream types of moisturizer, and oily skin would benefit more from gel type of moisturizer.
